I haven't bought Miva call inventory but wanted to respond to you.
Miva uses the Ingenio PPCall platform which is a bidded, keyword to category based ad serving application.
Miva and Ingenio distribution networks are unique, however. Whereas Ingenio serves call ads on AOL serps, Miva does not. Miva is instead focused on the FindWhat and Espotting assets. Miva does however sport a utilitarian network of sites of its own where call ads are apparently placed.
If you are in the States, SuperPages and Ingenio would be the first stop for call.
Miva may Looksmart but I question whether it will Enhance your current media plan. Do let us know. Thanks.

My brother has an account with both Ingenio and Miva. He reports that Miva is easier to deal with right now because Ingenio is still working out kinks with AOL. They are also going through a process of determining who can advertise in what categories which includes for now limiting individual advertisers from grabbing multiple local markets and keywords. Miva is less restrictive so he is able to cover more categories. As a result he gets more calls from the Miva side.
He loves it and who wouldn't, he has the top spot of any category he bids on!
Miva supplies pay per call to BizJournals.com and other affiliates. They will begin selling the ads in Europe soon according to a press release from the company and commentary I recently read here.
