Where to start with KW research?

What is, in your opinion, the best approach to start SEO Keyword research for website optimization?

Should I look first into the site, do a keyword density report and take it from there (wordtracker, etc.) or go directly into WordTracker and seek about website related keywords with best KEI?

Is there any SEO rule on this or it really depends on the website?

First brainstorm a list, then use Wordtracker to confirm. Forget about KEI.
