The title element is one of the most important on-page factors for SEO so it should be well thought out. I usually start by answering the question, "What is the page about?" in concise form. Definitely use keywords in the title but don't overdo it.
Also, keep in mind that the title element works hand-in-and with the meta description. I usually use an expanded version of the title as the description.
One very important note: Besides a title being effective for search engines make sure the title and description are attractive to the searcher. If the page does appear in the SERPs a good title and description help to "get the click."

do nnot use your company name unless it is a keyword.... use three keyword phrases per page is my rule of thumb

Something that catches the eye, describes the content on *that* page, etc.
Also, put specific information first, and then put generic information, like company name, *last*.
is this one correct?

Sounds good, phoenix.
The use of the company name or not -- branding vs. non-branding -- basically depends on your objective. I usually do use it (and yes, as the last element in the title), because I think that if people do repeated searches for related information and they keep seeing the brand at or near the top of the SERPs it will help to get the click.
Other folks do not use it, sometimes preferring to use that space as part of their "sell" title and a variety of other reasons.

Think of it this way... the first 6 or 7 words are shown in Search Engine results. The Search Engine wants those to be useful for the results, so structure the title with the first few words as the most important for results.
