Using forms on lending site

Hi,

My name is Peat and I run a website that deals with loans.
Up till now I've used loan information contant and was affiliated with loan companies presented by CJ. Up till now I've used banners and text adds.
Now I would like to enter a form on my website for sending loan companies the customers details directly and get a direct commission. I would like to know from someone that used this operation. - what are the steps to take and can I do it using CJ and if so, how?

To do that you need to have the company that does that type of marketing... then you put the form on your site and get paid for each one that gets filled out... look for email affiliate programs
