For years my client has been #1 in his particular category, but he wants to make major changes to the site now (it is over 5 years old).
He has an "Enter" page which brings on to his site. Would it be possible for the Enter button to refer to another site completely, which is the new site?
Would Google still pick up the other pages as they were before and assume that the Enter button is just a link to an affiliate, for example.

Yes u can do it.
There is a URL rewriting facility through which u can redirect the link displayed in SERP to ur new site. (Since in SERP still your old Enter button page link will be displayed).

That is all Google sees the link as... the thing is you may still want a link to that other page as it could hold links to other pages that link back to the page that ranks #1.
If he is just adding more content and revamping the site than he should leave the page as is and where needed have 301 redirects for the inner pages... the cavaet being if those pages all link to the other #1 page and will not when redirected then they will lose.
Part of the reason the #1 rank is from inbound links which are also pages from the site itself.
