Has anyone ever had a page get out of supplemental index?

I know why the pages are there. It is because the page hasn’t been touch in a long time or it doesn’t have any back links to it.

Has anyone had a page get out of the supplemental results without changing page names and using 301 redirects?

Yes I have seen pages that were stale and assigned to the supplemental index, revert back to the main index soon after the content was updated.

'Supplement Result' is shown due to following reasons:
1. As you mentioned, page didn't updated after some consecutive crawling
2. If page no longer working in real
3. Duplication Pages
